Townmark is an operator-led healthcare holding company partnering with rural primary care physicians. Through Townmark Health, we acquire and operate independent clinics (we don't flip them), building long-term homes for practices, patients, and the staff who hold them together.

Our Approach

We believe rural primary care is worth building around.

Independent rural practices are harder to run every year: administrative load, payer complexity, recruitment, regulatory change. Most consolidators answer that pressure by absorbing clinics into a hospital system or stripping them for scale. We work differently. Our model is built to let physicians keep practicing in the communities they've served, while we take the operating weight off their shoulders.

A good partnership leaves the clinic feeling more like itself, not less. Here's what stays the same after we partner with a practice, and what we take on so the clinical team can focus on patients.

What Stays the Same

The practice patients trust.

  • The clinic keeps the name patients know. Townmark is the parent company; Townmark Health is the operating platform behind the scenes, not the marquee.

  • Physicians and clinical staff continue practicing as they do today. No outside party directs clinical decisions.

  • MAs, nurses, front office, and longstanding staff stay. Continuity is part of what we're paying for.

  • Same providers, same panels, same continuity of care. We don't redirect referrals or restructure access.

  • No forced platform migrations. We integrate around the systems already in place.

What We Take On

The work that shouldn't be a physician's job.

  • End-to-end billing, denials, follow-up, payer issues, and posting, run by a centralized team, not by your front desk.

  • Commercial, Medicare, and Medicaid contracting handled centrally. Provider enrollment and re-credentialing managed for you.

  • Centralized expertise in rural health regulation, including federal designations that meaningfully improve reimbursement where a clinic qualifies.

  • CCM, RPM, and related programs run by central teams: incremental revenue without adding local headcount.

  • Eligibility checks, claim status, dashboards, automated posting, integrated alongside what you use, not in place of it.
